1. Acceptance of Terms
Welcome to Polaris Medicare. These Terms of Use ("Terms") govern your access to our website and the clinical home care services, telemedicine platform, and equipment rentals provided by us. By booking a service, visiting our website, or enrolling in a care package, you acknowledge that you have read, understood, and agree to be bound by these Terms.
If you are registering for care on behalf of a family member, parent, or dependent, you warrant that you hold the legal authority to agree to these terms on their behalf.
2. Clinical & Medical Disclaimer
🚨 Critical Medical Disclaimer:
Polaris Medicare provides transitional home-based care, post-surgical recovery support, and therapeutic assistance. Our home care services and telemedicine consultations do NOT replace acute hospital emergency departments. In the event of a sudden, life-threatening clinical crisis, chest pains, severe breathing problems, or sudden physical trauma, contact local emergency ambulance services or visit the nearest hospital emergency room immediately.
Our telemedicine platform is designed to provide general consultation, follow-up triage, and chronic monitoring. It is not an alternative to in-person clinical examinations when physical symptoms require direct diagnostic assessment.
3. Description of Services & Clinical Protocols
We coordinate home healthcare services which are delivered by trained, qualified professionals. Our services include:
- Nurse Care at Home: Vitals tracking, medication administration under physician directives, wound care, and post-hospitalization monitoring.
- Physiotherapy at Home: Restorative physical therapy plans for orthopedic, stroke, or respiratory rehabilitation.
- Telemedicine & Diagnostics: Remote video consultations and blood collections performed by certified phlebotomists.
- Medical Equipment: Hospital-grade equipment rentals (e.g. ICU beds, oxygen concentrators) which remain the property of Polaris Medicare or our suppliers.
4. Patient & Family Responsibilities
To ensure high standards of care and guarantee the safety of our visiting clinical staff, patients and families agree to:
- Provide accurate, complete, and truthful medical history, past diagnoses, and active prescription details.
- Ensure a safe, sanitary, and respectful environment for our visiting nurse practitioners, physiotherapists, and care workers.
- Adhere to the medication and treatment guidelines prescribed by the patient's consulting physician.
- Notify Polaris Medicare coordinators at least 12 hours in advance in the event of schedule changes, hospital admissions, or visit cancellations.
5. Equipment Rentals & Damage Liability
All medical equipment rented to patients is checked for calibration and functionality before delivery. Rentees agree to:
- Operate the equipment strictly as directed by the manufacturer and our technical specialists.
- Maintain the equipment in clean, secure conditions. Any structural or electronic damage due to negligence, misuse, or external power fluctuations will be billed to the client.
- Return the equipment promptly at the end of the rental term. Late return fees will apply if rentals are not returned or extended officially.
6. Billing, Payments & Cancellations
Home healthcare packages must be paid in advance or in accordance with the billing cycle specified in your service contract. If a scheduled practitioner visit is cancelled by the family with less than 2 hours notice, a nominal visit fee may be applied to cover transportation and resource allocation.
